To ensure a safe and positive experience for all participants, Mount Pleasant Community Development Corporation (MPCDC) requires a completed release form for certain programs and activities. Please download and complete the form for your program. You may fill it out electronically (fillable PDF) or print and hand-sign.

How to submit: Email the completed form to info@mpleasantcdc.org or bring a printed copy to your program check-in.
